    Tapestry 2021 Artisan Contract

    Please complete this form to apply to become a Tapestry artisan. The completed form contents will be used for all Tapestry promotional information.

    Check Payment: Following submission of this contract, mail photographs and payment of $100 via check made payable to “Purdue Fort Wayne” to the address below.

    Credit Card Payment: 
    Following submission of this contract, you will receive an email from Tapestry with instructions on how to submit a secure credit card payment using the Purdue Fort Wayne Payment Portal. Digital photographs should be emailed to tapestry@pfw.edu. 

    Payment of $100 must be made in full for your application to be processed.
    Agreement
    1. All Artisan applications are due June 10, 2021.
    2. It is a juried event and all Artisans will be notified of participation by June 30, 2021.
    3. If you are put on the ‘Wait List’ you will be notified if booth space becomes available.
    4. Mandatory booth set-up is from noon until 5 p.m. on Thursday, September 16, 2021.
    5. Artisan participants are required to be at Tapestry from 7:15 a.m. until 4:30 p.m. Friday, September 17, 2021.
    6. Masks are required for everyone in attendance including Artisans, sponsors, booth workers, attendees, volunteers, and all individuals on premises both during set-up day and event day.
    7. Hand sanitizer stations will be provided, however it is highly encouraged to bring a personal supply for individual use. It is encouraged to have a larger courtesy bottle for shoppers to use as well. 
    8. Each booth space may only have two workers at a time, including the artist/artisan.
    9. The 10’x10’booth will have one chair and one 8’clothed and skirted table. Additional tables may be rented.
    10. Each booth space is defined by an 8’curtain backdrop and 8’side curtains. This means that each booth will have three sides with 8’ pipe and drape (rather than the 3’ foot side walls as in the past).
    11. Electrical outlets are available at $30 per outlet. A separate electrical form must be completed prior to the day of the show to obtain electricity in your booth.Please note that electrical outlet requests on the day of set up cost $50 and $100 on event day.
    12. No outside food or drink is permitted. Individual box lunches are available for $15.
    13. Watch for details in the Booth Information Packet that will be sent in July.
    14. It is the sole responsibility of the Artisan and all booth workers to follow these guidelines.
    15. Tapestry reserves the right to dismiss an Artisan that is not following said guidelines and any new guidelines should any arise between the date this contract is submitted and event date.
    16. By submitting this application, the Artisan assumes all COVID-19 risks associated with attending Tapestry.
    17. By submitting this application, the Artisan has entered into a binding agreement with Purdue University Fort Wayne and Tapestry and guarantees payment and participation in the Tapestry event. A cancellation on the artisan’s behalf will be considered a donation to the Tapestry Scholarship Fund.
    18. If Tapestry is transitioned to a virtual event or cancelled in its entirety, Tapestry will contact you via email with instructions and deadlines to initiate a donation and/or refund process. It is the responsibility of the artist/artisan to monitor their email to remain informed and take appropriate action within schedules provided.
